For me, we have:
Rock A: The Bean Counter, or Missing Missile (for both senses of 'missing').
Rock B: Kalian, or when used for unlocking the Crystal Cruiser - Crystal Hunter.
Zoltan A: Judgypants.
Zoltan B: No Soap Ether, or Artinator.
Engi A: Glazed Death
Engi B: Flushed
Kestrel A: Space Chicken
Kestrel B: Back to Basics
Slug A: Slug of War
Slug B: Stormcrawler
Crystal A: Lattice, or Diamonds Forever, or when used to unlock the B Layout - Carnel Desire
Crystal B: Crown Jewel
Stealth A: NowYouSeeMe
Stealth B: Glaive Grave. I used it to try to attempt getting the 'Never Saw it Coming' achievement, naming it 'Prelight my Fire'; first battle I received the Preigniter Augment. The RNG is all-knowing, most merciful.
Mantis A: The Gilat Pet
Mantis B: The Loveboat, or Four-by-Four (because it's a board. I kill me).
Federation A: The Laser Spray
Federation B: Charybdis
I usually refrain from naming crew, since any theme is lost with new members and reassignments (the best one could hope for, it seems, would be to name everyone 'Charlie'). The few exceptions were my first games, were I named the crew 'Capn', 'Boomer', and 'Dodger' (bonus points for guessing their positions), and when as a beginner I had trouble with the Adjudicator, their crew were named 'Shortlived', 'Cursed' and 'Doomed' (Doomed held the record for longest jumps survived for a long while) while the ship was called 'Space Confetti'. Lastly, Engi B's plucky member is always named 'MrLonely'.

I leave my ships named what they're named.
When recording, I name my starting crew after people who have asked on YouTube.
When not recording, I leave the crew named what they're named.

Usually, my laziness trumps my creativity, and I don't name my ships unless I'm using them repeatedly for a specific purpose. Ships going for unlocking the Mantis cruiser were named Mantis Maimer and Mantivora et al., ships going for unlocking the Crystal cruiser were named after various jewels. The third Crystal cruiser ship achievement was obtained by Rocky V.

Those aren't renamed ships. Those are ship mods. I'm pretty sure that Blasteroid was just the name of the Asteroid ship before it was completed and released. (I could be wrong.) You are almost correct. Blasteroid was a different ship than the Asteroid. I don't think it had a teleporter by default. Ah, I didn't know that these ships are mods. Never played mods and probably never will, as they make the all-important scoring system arbitrary and make you lose the frame of reference.

I love renaming the ships and crews.
I used to always theme the ship and the crew names every time I played, but now I mostly only put the time into coming up with a full theme if I'm recording the run. I do still almost always rename the ship to something (and almost always start it with "VSS" for valorous space ship for whatever reason) though, even if I'm just playing on my own.
It makes me more invested in the runs, and I care more about the survival of my starting crew that way. It doesn't really bother me that the theme is broken as soon as we get more crew, and that actually makes it easier a lot of the time because I don't need to think of a theme that can accommodate 8 components.
I definitely don't always use the same kind of names for each ship with any kind of consistancy though.

One run, I had 3 crew members all named Charlie (a Zoltan, a Rock, and an Engi, I think).
