My biggest fail was when I got incredibly lucky playing with the Red-Tail. I managed to pick up three engis, two mantises, a Burst Laser II, a Glaive Beam, an Ion Bomb, and a Heavy Laser I. I had max shields, nearly max engines, plus a teleporter, cloak, and drone control with a defense drone I.
After reaching the third stage of the flagship for the first time ever, I was ecstatic, absolutely certain that I would beat the rebels for the first time. I destroyed the zoltan shield, used the ion bomb on the shield room, teleported into the missile room, and managed to use my burst laser to get rid of the two remaining shield layers and destroy the shield system. I was doing brilliantly.
However, in stage two, I had made a fatal error: the only crew member I had left alive was the one manning the beam weapon, and, this being my first time to beat stage two, I hadn't realised there was a third stage and that the starboard (right) wing broke off.... The AI had taken over by stage three - I planned to keep my two mantises (mantii?) in the main segment of the flagship to undo any repairs the AI made. However, just before the mantises destroyed the missile launcher, the boss sent three missiles and it's triple heavy laser at me. They destroyed my cloak, shields, and plenty of other systems (can't remember which.) Just after that, it regenerated it's zoltan shield, preventing me from bringing back my boarders from the missile room. I sent an engi to repair the shields. It managed, but after it had done so, the power surge happened. Innumerable heavy lasers came my way, it felt like they were travelling across the screen in slow motion. With my cloak destroyed, my shields down after another triple heavy laser attack, countless other systems in need of repair, and my mantises trapped, all I could do was scream "Noooooooooooooooooooo!!!!" as the Red-tail exploded into smithereens, firing one more heavy laser volley at me even after I had begun to explode just to hammer the point home. Was there anything I could have done?

edit: Alternately, don't put the fires out, or turn your oxygen off, or turn your shields off during the combat section. There are a lot of ways to die. None of them can ever happen by accident, because the tutorial is well designed, but you can die.
edit2: Actually, that's not entirely true. The first method (all doors open) has killed me a few times when I've misclicked without pausing.

Actually I very nearly died once because my oxygen level was slowly decreasing... I couldn't work out why... At the very last second I realised that the oxygen system wasn't even powered.

Forgot to re-power oxygen and constantly wondering where there are breaches and why I'm dying. Well, at least that run was not that good anyway.

I had a wonderfully powerful crew that I used to wreck every ship by teleporting. Well, I was attacking the mothership and was crushing it but I wanted to slow down so I could take out several of the opposing crew to make the next steps easier. I beamed over my 4 fully upgraded characters just in time for my artillery beam from my Federation Cruiser to set off and destroy the cruiser. And without my 4 crewmembers I was unable to take out the various systems in the next fight and was destroyed. The facepalm I did after that mistake was epic.
