My most common one would be forgetting to pick up my boarding team after a successful boarding action and FTL jumping out, bye-bye mantis crew members.
For some reason I always forget I can FTL away from a bad fight and instead fight to the death

I jumped away from a battle with a completely destroyed shield system, into the refugee ships event. The run was going pretty good despite that hiccup, and the sensible thing to do, obviously, is avoid them and patch up.
But being completely stupid, I hail them, get into a fight, and doom myself.

I've done some stupid things in FTL, but the single most common one is this -
Battle against AI Drone that has too many shields.
Has Lvl 2 Teleport, so I can damage systems and save my guys.
Manage to offline shields/weapons/drones.
Sigh of relief.
Move my mouse to click on beam back.
Drone cloaks.
Tears.
I have done this perhaps once every four or five runs since I started using the teleport and it is about the only thing guaranteed to engender a total ragequit.

kill all the crew on flagship by mistake ( read: BY FIRE, BURN, BURN TO THE GROUND! ) in p1, therefore dying horribly in p2
